About the role
We are recruiting an experienced Outreach Worker to support rough sleepers and vulnerable adults experiencing homelessness, helping them access essential services and secure suitable accommodation.
Location: Chelsea & Kensington
Contract: Temporary, Ongoing
Hours: Rolling rota, 7 days a week
Key Responsibilities:
Engage and support rough sleepers and vulnerable adults.
Complete needs and risk assessments and support plans.
Assist with accommodation, healthcare, benefits and referrals.
Work with local authorities and partner agencies.
Maintain accurate case notes and follow safeguarding procedures.
Promote independence and long-term housing solutions.
Requirements:
Experience in homelessness, outreach or supported housing.
Knowledge of safeguarding, risk assessments and complex needs.
Strong communication and relationship-building skills.
Ability to work independently and manage a caseload.
