We are seeking a compassionate and experienced Adults Mental Health Social Worker to join our team in Sheffield. The ideal candidate will have a strong background in mental health social work, excellent interpersonal skills, and a commitment to supporting adults with mental health needs. This role involves providing comprehensive case management, conducting assessments, and working collaboratively with multidisciplinary teams to ensure the best outcomes for service users.
Responsibilities
- Manage a diverse caseload, providing ongoing support and intervention.
- Coordinate and monitor the delivery of services to ensure effective and timely support.
- Maintain accurate and up-to-date case records and documentation.
- Conduct comprehensive assessments of adults with mental health needs.
- Develop and implement individualised care plans in collaboration with service users, their families, and other professionals.
- Regularly review and update care plans to ensure they meet the evolving needs of service users.
- Provide emotional and practical support to adults with mental health needs.
- Advocate for the rights and needs of service users within the community and health care system.
- Facilitate access to resources and services, such as housing, health care, and financial assistance.
- Identify and respond to safeguarding concerns, ensuring the safety and well being of vulnerable adults.
- Follow local safeguarding procedures and policies.
- Report and document any concerns or incidents promptly.
Education: Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution.
Licensure: Registered with Social Work England.
