- About The Role - Reablement Day Centre Occupational Therapist
We are recruiting for a Reablement Day Centre Occupational Therapist to join our Reablement Team.
This is an exciting role, managed within Reablement but working closely with Sundial Day Opportunities (with plans to bring Sonali Gardens into the remit in the future) as part of a new model of day care. This model focuses on reablement and "normalising" leisure and social activities, moving away from the traditional day centre model.
This is a developing role and there will be opportunities for the successful candidate to shape the service whilst supporting service users and day centre staff (at Sundial and, in the future, Sonali Gardens) to explore exciting new opportunities. The role will empower people to take control of their own leisure activities and participate in activities that are truly meaningful to them.
You will work alongside the Reablement Clinical Lead to develop the service. You will support day services to work within an enabling ethos by completing functional assessments and developing plans to help service users transition into universal services, maintain functional ability, and identify meaningful activities to participate in.
The role incorporates both group work and one-to-one interventions and involves supporting day centre staff to adopt an enabling approach.
Role Purpose and Aims
- Oversight of group activities and activity planning across day hub sites and community hubs, ranging from service users requiring short-term day support input with a reablement focus and a view to moving into universal services, through to those with high support needs.
- Use of Reablement Officers (ROs), where appropriate, to support people in achieving their goals.
- Development and delivery of evidence-based groups, supported by evidence-based group protocols.
- Supporting day centre staff to develop and facilitate groups within community settings.
- Clinical oversight of goal setting for individuals with the potential to transition to universal services following 3-12 months of day service input.
- Clinical support for less complex goal setting.
- Completion of ad hoc functional assessments without the need for referral to Community Therapy services.
- Working closely with the Independent Living Hub to identify assistive technology (AT) and equipment solutions to maximise attendees' independence within day services.
- While the role currently covers Sundial (as Sonali Gardens has not yet transitioned into the new model), 0.5 of the role will be based within Sundial and 0.5 will involve carrying a standard Reablement OT caseload. This provides an excellent opportunity to gain experience in reablement, supporting people to maximise their independence and achieve their goals with the support of Reablement Officers.

We are looking for someone who can demonstrate:
- A Degree in Occupational Therapy and HCPC registration
- Experience in:

- Group facilitation.
- Strong communication and collaborative skills, with the ability to work effectively within a diverse community.
