We are seeking Community Care Officers (Level 2) to join our team.

About the Role

As a Community Care Officer, you'll work with adults who may be experiencing physical disabilities, long-term health conditions, frailty, or other care needs. You will be working with people living in a variety of settings, including their own homes, supported accommodation, residential care and nursing environments.

Working within Lincolnshire's strengths-based Conversation Model, you will:

  • Complete Adult Needs Assessments and reviews under the Care Act 2014
  • Develop strengths-based support plans that focus on individual outcomes
  • Work alongside individuals, carers, families and partner agencies
  • Promote independence, wellbeing and community connections
  • Respond to safeguarding concerns and contribute to safeguarding enquiries
  • Support people through periods of change, crisis and transition
  • Manage a varied and rewarding caseload across your locality

The Area Team focuses on working with adults who require assessments, care planning, and ongoing interventions to meet their care and support needs. This often involves undertaking comprehensive assessments, developing person-centred support plans, and working collaboratively with families, carers, health professionals, and other partner agencies. The team covers Louth, Market Rasen and the surrounding areas.

The Review Team focuses on completing planned reviews for people who already have care and support services in place, ensuring that arrangements continue to meet their needs and desired outcomes. The team also supports people following discharge from hospital, helping them adapt to changes in their circumstances and reviewing support where required. The team is based across Louth, Market Rasen, Boston and Skegness.


Currently, we have two vacancies available:

  • 1 full time Community Care Officer – Louth & Market Rasen Area Team (Louth office)
  • 1 full time Community Care Officer – East Review Team (office base can be Louth, Boston or Skegness)

This is a community-based role so you must have the ability to travel across the county in order to meet the needs of the service.
