Pay Rate: £28.40ph Umbrella Hybrid Arrangements: 2-3 days in office dependent on demand May need to travel to out of area hospitals if required The post holder is directly accountable to your Senior Social Worker who is in turn accountable to the Redwoods Mental Health Social Work Team Manager and AMHP Lead. Duties will involve contact with staff throughout the Council, customers, carers and with staff of other agencies, including statutory and voluntary services. You will carry out your work to the standards set in the relevant professional and national occupational standards. - To carry out duties relating to the local authority statutory requirements under The Care Act (2014) and The Mental Health Act (83/07) and The Mental Capacity Act (2005) - To Manage a caseload of individuals with complex mental health needs, supporting timely hospital discharge and addressing any barriers to discharge. - Section 117 aftercare planning and reviews, ensuring appropriate aftercare services are arranged where eligible. - Arrange and coordinate care packages, placements, supported accommodation and community support services to enable safe discharge from hospital
