About the role
Youth and Community Support Practitioner
Location: Telford and Wrekin
Hours: 37 hours (12 months fixed term)
24 Hours (12 months fixed term)
18.5 Hours (Permanent)
Are you passionate about supporting children, young people and families to achieve positive outcomes? Do you have experience delivering community-based support and working collaboratively with partners to strengthen local services?
We are looking for a Youth and Community Support Practitioner to join our dedicated team, helping to deliver responsive, evidence-based support to children and families across Telford and Wrekin.
About the Role:
As a Community and Youth Support Practitioner, you will be responsible for delivering a range of direct support interventions to children, young people, and families. Support will be needs-led, evidence-based and delivered within schools, local communities, and family homes.
Working closely with the Senior Practitioner and Team Leader, you will respond to identified family needs through both group and individual interventions, helping families access the right support at the right time.
You will play a key role in building strong partnerships with schools, community organisations, voluntary sector partners, and other stakeholders, helping to develop a culture where community-based responses are valued and recognised as a first point of support.
