Vitalis are recruiting for experienced Social Worker to work in a Transitions Team in Salford Council.

To support this ambitious programme and continue delivering high-quality services for our residents, we are looking for experienced and motivated Advanced Social Work Practitioners to join our Front Door Services.
This is an exciting opportunity to work across both our:

• Early Help Team
• Safeguarding Team
• Working across both Early Help and Safeguarding, you will be instrumental in providing timely, proportionate interventions that promote independence and wellbeing. Using a strengths-based approach, you will support adults to delay, reduce, or prevent the need for ongoing formal services, while ensuring people remain safe, empowered, and at the centre of decision-making, achieving the outcomes that are most important to them.
