About the Role

Bluestones Medical is currently recruiting experienced Community Nurses (RGNs) to support Betsi Cadwaladr University Health Board across community hospital settings throughout Wrexham, Flintshire and Denbighshire.

As a Community Nurse, you will provide high-quality, person-centred care to patients within community hospital environments, supporting recovery, rehabilitation, long-term conditions management and complex care needs. You will work alongside multidisciplinary teams to ensure patients receive safe, effective and compassionate care closer to home.

Key Responsibilities

• Deliver high-quality nursing care to adult patients within community hospital settings.
• Assess, plan, implement and evaluate individualised care plans.
• Support patients with a range of medical conditions, rehabilitation needs and long-term illnesses.
• Administer medications, treatments and clinical interventions safely.
• Monitor patients’ conditions and escalate concerns appropriately.
• Support discharge planning and promote independence where appropriate.
• Provide wound care, catheter care, medication management and other clinical interventions.
• Maintain accurate and timely patient documentation.
• Work collaboratively with community teams, GPs, therapists and wider multidisciplinary professionals.
• Promote dignity, choice and patient wellbeing at all times.
• Follow NMC standards, infection prevention guidance and BCUHB policies.

Essential Requirements

• Registered General Nurse (RGN) with a valid NMC Pin.
• Minimum of 6 months’ recent UK nursing experience.
• Experience working within community hospitals, rehabilitation, medical or elderly care settings desirable.
• Strong clinical assessment and care planning skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Right to work in the UK.
• Ability to provide satisfactory employment references.
• Up-to-date mandatory training (or willingness to complete).
