QSW (Qualified Social Worker)

We are seeking an experienced Social Worker to join Cornwall Council’s Adults Community Team on a 3-month locum contract. This role will involve working with adults in the community to assess needs, plan and implement care packages, and ensure the delivery of person-centred support in line with statutory requirements. The successful candidate will be committed to promoting independence, safeguarding vulnerable adults, and working collaboratively with multi-disciplinary teams.

Key Responsibilities:

• Undertake Care Act assessments to identify individual needs and determine appropriate support plans.

• Carry out Mental Capacity Act assessments and participate in Best Interest decision-making where required.

• Manage a caseload of adults in the community, ensuring timely interventions and reviews of care packages.

• Work collaboratively with health professionals, voluntary agencies, and other stakeholders to deliver coordinated care.

• Identify and respond to safeguarding concerns in line with local authority policies and procedures.

• Maintain accurate and up-to-date case records in compliance with legislative and organisational requirements.
