Job Overview

Information and Assessment Officer required to support Adult Social Care services, delivering high-quality, customer-focused information, advice, and initial conversations to Hackney residents and their carers.

Key Responsibilities

•  
  • Work with Social Workers and other professionals within Adult Social Care to provide effective, accessible information and Initial Conversations.
     
  • Deliver a high standard of customer care, offering advice and assistance on council services and services provided by statutory, independent, and voluntary agencies.
     
  • Carry out initial conversations using a strengths-based approach to identify needs and promote independence.
     
  • Act as a Trusted Assessor, providing simple equipment to older and disabled people to help them maintain independence.
     
  • Produce individually tailored plans for simple equipment provision.
     
  • Recommend and requisition Telecare services where appropriate.
     
  • Support the setup of social care services following assessment.
