  • Support both formal and informal carers with practical guidance, education, and advice.
  • Work collaboratively with multidisciplinary professionals, health services, housing, and community partners.
  • Manage a varied community caseload, prioritising work according to clinical need.
  • Maintain accurate, timely, and legally compliant case records and professional documentation.
  • Contribute to service development and continuous improvement initiatives.
  • Support strengths-based, person-centred practice that places individuals at the centre of decision-making.

About You

You will be an experienced Occupational Therapist with a positive, collaborative approach and a passion for supporting adults to live as independently as possible within their own communities. You will be confident managing community caseloads, undertaking complex assessments, and working effectively within a multidisciplinary team.

You will enjoy contributing to service development and will welcome opportunities to broaden your skills through supervision, mentoring, and exposure to specialist areas of practice.

Requirements

  • Current HCPC registration.
  • Experience working within Adult Community Occupational Therapy or Adult Social Care.
