Are you passionate about helping people maintain their independence and making a real difference in your community? If so, we’d love to hear from you.

We are looking for an enthusiastic, proactive and compassionate individual to join our newly established Access Team. This is an exciting opportunity to play a key role in supporting adults to live independently, achieve their goals, and access the help they need when they need it.

In this role, you will manage a varied caseload, undertaking strengths-based assessments, developing and reviewing support plans, coordinating services, and ensuring our safeguarding responsibilities are met. You will work closely with individuals, their families, carers, and partner organisations to deliver person-centred support that focuses on what matters most to each person.

The Access Team provides early intervention and practical support to help people remain independent and prevent or reduce the need for crisis services. This may include assessing and arranging low-level mobility aids, supporting people with housing, finances, medication management and appointments, assisting with home management tasks, and arranging access to essential items such as emergency food parcels.

We are looking for someone who is passionate about empowering people to make their own choices and retain control over their lives. You will share our commitment to strengths-based practice, recognising and building on the skills, abilities, relationships and community networks that people already have. We are embedding the Three Conversations model within Adult Social Care and are seeking individuals who are excited about being part of this innovative approach.

You will have excellent organisational and communication skills, with the ability to work effectively alongside a range of professionals and partner agencies, including health colleagues and community organisations. A demonstrable commitment to person-centred, strengths-based practice is essential.

We are committed to supporting the growth and development of our workforce. You will benefit from regular supervision, ongoing training, career development opportunities, and the support of a motivated and dedicated team.

Applicants must hold a valid driving licence and have access to a vehicle for work purposes
