Personal Advisor – Leaving Care
Location: Peterborough – Sand Martin House / Chauffeurs Cottage
Working Pattern: Hybrid
Role Overview
Peterborough City Council is looking for an experienced Personal Advisor to join the Leaving Care Service, supporting young people as they transition from care into adulthood and independent living.
The successful candidate will manage a caseload of care leavers, providing practical and emotional support, completing assessments and reviews, developing Pathway Plans and working with partner agencies to improve outcomes for young people.
Key Responsibilities
- Manage a caseload of individual care leavers in line with relevant legislation, policies and procedures.
- Assess the needs of young people and provide appropriate support and intervention to help them achieve positive outcomes.
- Develop, plan and review Pathway Plans with care leavers, ensuring young people remain at the centre of all planning and decision-making.
- Support young people with accommodation, education, employment, training, finances, health and independent living.
- Assess financial needs and support care leavers with budgeting for accommodation and maintenance.
- Undertake regular visits to young people in their homes and other appropriate settings.
- Maintain accurate and confidential case records and ensure all documentation is completed in line with departmental standards.
- Attend and contribute to reviews, planning meetings, case conferences and other professional meetings.
- Work collaboratively with Children’s Social Care, education, health, housing and other partner agencies.
- Identify and respond appropriately to safeguarding concerns involving vulnerable young people.
- Provide services in accordance with the Children Act and Leaving Care legislation, guidance and regulations.
- Promote equality, diversity and social justice throughout all aspects of practice.
- Work flexibly to meet the needs of young people, including lone working and occasional unsocial hours.
- Participate in supervision, appraisals, practice observations and ongoing professional development.
- Maintain up-to-date knowledge of relevant legislation, policies, procedures and best practice.
About You
The ideal candidate will have experience of working with care leavers, vulnerable young people or within Children’s Social Care and be confident managing a caseload independently.
You will need strong assessment, communication and partnership-working skills, together with an understanding of safeguarding and the challenges young people can face when transitioning into adulthood.
Experience of Pathway Planning, leaving care legislation and supporting young people towards independent living would be highly desirable.
