Experienced Qualified Social Worker - Adults

To provide a high-quality social work service to adults and young people with learning disabilities, physical disabilities, and neurological conditions, promoting independence, wellbeing, choice, and control.

Possession of full driving licence, access to a vehicle and willingness to travel both inside and outside the council area as required.

Undertake strength-based assessments of need, risk, and eligibility under the Care Act 2014 and other relevant legislation.
Develop, implement, monitor, and review person-centred support plans that promote independence and wellbeing.
Complete Mental Capacity Act assessments and Best Interest decision-making processes where appropriate.
Carry out Continuing Health Care assessments and contribute to multidisciplinary assessments.
Support effective transition planning for young people moving from children's to adult services.
Manage a complex caseload involving individuals with multiple and complex needs.
Safeguarding Lead and undertake safeguarding enquiries under Section 42 of the Care Act.
Identify and manage risks, balancing duty of care with individuals' rights to make informed choices.
Work with individuals, carers, providers, and partner agencies to protect adults at risk from abuse and neglect.
Prepare reports and provide evidence for safeguarding meetings, reviews, and legal proceedings where required.
Direct Work and Intervention Build effective relationships with individuals, families, carers, and advocacy services.
Undertake person-centred interventions that promote empowerment, resilience, and independence.
Support individuals experiencing significant life changes, deterioration in health, or crisis situations.
Advocate for individuals to ensure their voice is heard and their rights are upheld.
Partnership and Multi-Agency Working Work collaboratively with health services, housing providers, education, voluntary organisations, and community resources.
Participate in multidisciplinary meetings and contribute professional social work expertise.
Foster effective partnership working to improve outcomes and reduce barriers to independence.
