Our Team:

The Reablement service provides short term care and support to adults within their own homes within the Halton area. The team focuses on service users strengths to enable them to stay independent and improve their wellbeing. Our team aims to build service users confidence after a stay in hospital, illness or deterioration in their health; preventing re-admission to hospital or a premature move to a care home.

The Reablement Service operates 365 days per year. Our care and support worker shift patterns are across a two week rota, working every other weekend. All posts include a mixture of early and late shifts. Early shifts commence at 7.30am and Late shifts finish at 9pm or 11pm.

Enhancements are paid for weekends and unsocial hours. You will receive a car allowance payment monthly in addition to your business mileage.

Your Contributions:

The Reablement team is looking for caring, hardworking, compassionate and innovative individuals who can ensure that service users care and support plans are provided to a high standard, empowering service users and promoting their independence. Working within a multidisciplinary team, you will develop and maintain effective working relationships with a range of professionals including our therapy and social work teams. Candidates must possess excellent interpersonal skills and have the ability to effectively communicate with and listen to others. You will also be competent in accurately maintain clear and concise written records and producing reports.
