About the role

You will be responding to those in need of social care support and undertaking the required work in accordance with the Care Act 2014 and Mental Capacity Act 2005.

About the Teams

Integrated Discharge Team - Watford General Hospital
37 hours per week | Permanent

The Integrated Discharge Team at Watford General Hospital support a 7 day discharge service. This is a 37 hour a week position, and 1 in every 8 weekends you would be expected to work a Saturday and Sunday.

Our work is fast paced as we focus on supporting people to leave hospital as quickly and safely as possible, with a person-centred approach, with the majority of the work being through Discharge to Assess.

The usual hours are 9am – 5pm and you would need to be able to work in Watford General Hospital every. Due to the nature of the hospital work, there is no remote or hybrid working in this role.


About you

Essential criteria

  • The team is community based and you must have a full UK driving licence and regular access to a vehicle for working purposes
  • Experience of working in social care
