18+ Leaving Care Personal Advisor
Birmingham Children’s Trust
Birmingham Children’s Trust is looking for an experienced 18+ Leaving Care Personal Advisor to support care leavers aged 18–25 as they transition from care into adulthood and independent living.
Key Responsibilities
- Manage an individual caseload of eligible, relevant, former relevant and qualifying care leavers.
- Complete, coordinate and regularly review Pathway Plans with young people.
- Provide ongoing one-to-one advice, support and practical guidance to help young people achieve independence.
- Assess individual needs and ensure young people receive their statutory entitlements and appropriate services.
- Support care leavers with areas including accommodation, employment, education, benefits and independent living.
- Work collaboratively with social workers, carers, parents and partner agencies to coordinate appropriate support.
- Support young people with continuing care needs and help ensure a positive transition into adulthood.
- Participate in duty and referral work, responding to young people’s needs as they arise.
- Maintain accurate and secure case records, assessments, plans and other casework using electronic and paper systems.
- Monitor and review the effectiveness of support provided and contribute to relevant reports and service development.
- Promote anti-discriminatory practice, equality and positive outcomes for care leavers.
- Keep up to date with relevant legislation, policy and best practice relating to leaving care, accommodation, employment, benefits and support services.
- Participate in regular supervision, training and relevant service development activities.
Requirements
The successful candidate will have:
- Experience of working with care leavers, looked-after children or young people transitioning to adulthood.
- Experience of case management, assessments and support planning.
- Knowledge and practical experience of Pathway Plans and leaving-care services.
- Experience working with multiple agencies to coordinate support.
- Good knowledge of issues affecting care leavers, including housing, benefits, education, employment and independent living.
- Strong recording, communication and organisational skills.
- The ability to work independently within established procedures while managing a caseload effectively.
- A commitment to safeguarding and promoting the welfare of children and young people.
Additional Information
- Reports to: Team Manager
- Supervision: Regular supervision with the Team Manager
- Flexible working hours may be required to meet the needs of the service and young people.
- This vacancy is exempt from the Rehabilitation of Offenders Act.
- A DBS/ISA check will be undertaken.
