Transformation Consultant /Hospital Discharge/ Reablement Lead

Morson have a great opportunity for an experienced Consultant that as worked within Local Government /Adult Social Care. We are seeking a talented and motivated Localities Lead for  our transformational initiatives across hospital discharge, locality teams, and reablement services.

Key Responsibilities:

• Lead and manage transformation projects aimed at improving hospital discharge processes and reablement services.
• Collaborate with local government departments, health services, and community organizations to create integrated and effective locality teams.
• Utilize strong analytical skills to assess existing processes, identify opportunities for improvement, and implement evidence-based solutions.
• Foster a culture of continuous improvement, ensuring that services meet the needs of the community and adhere to best practices.
• Engage stakeholders at all levels to promote collaborative working and to champion change.

Essential Qualifications:

• Extensive experience in local government, with a thorough understanding of health and social care systems.
• Proven ability to analyze complex data and translate insights into actionable strategies.
• Strong leadership skills with experience in managing teams and driving change.
