Pertemps are currently recruiting for an experienced Adult Social Worker to join a busy Hospital Discharge Team based in Aylesbury.
This is an excellent opportunity to play a key role in supporting timely and effective hospital discharges, working closely with health professionals, service users, families and partner agencies to ensure safe and appropriate outcomes.

Pertemps are currently recruiting for an experienced Adult Social Worker to join a busy Hospital Discharge Team based in Aylesbury.
This is an excellent opportunity to play a key role in supporting timely and effective hospital discharges, working closely with health professionals, service users, families and partner agencies to ensure safe and appropriate outcomes.
Key Responsibilities
- Managing a complex caseload within a fast-paced Hospital Discharge Team
- Completing Care Act assessments, reviews and support planning
- Assessing service users to identify appropriate care and support arrangements
- Supporting safe, timely and effective hospital discharges
- Working collaboratively with multidisciplinary teams, including NHS colleagues
- Liaising with families, carers and external agencies
- Completing Mental Capacity Assessments and Best Interest decisions where required
- Ensuring safeguarding concerns are identified and appropriately managed
- Maintaining accurate and timely case records
- Qualified Social Worker with Social Work England registration
- Previous experience within Adult Social Care
- Experience of hospital discharge or a fast-paced statutory social work environment
- Strong knowledge of the Care Act 2014 and relevant adult social care legislation
- Ability to manage a complex caseload and work to deadlines
