Hours: 6 sessions a weekminimum - 3 full days compressed option available
Annual leave: 6 times your number of weeklysessions (36 sessions)
Study leave: Up to your weekly number ofsessions (3-5 days)

Weare a GP Training and Medical Student teaching practice in East Ham, Newham,looking after over 13,000 patients from very diverse backgrounds. Our work isalso diverse. We pride ourselves on providing high quality services and we arekeen on learning and developing new ways of working. We are part of the biggestPrimary Care Network (PCN) in Newham and our CQC rating GOOD.
As one of the larger practices in Newhamwe have 5 GP Partners, 2 Clinical Pharmacists, 1 Nurse, 3 Health CareAssistants and 2 Pharmacy Technicians. We have a named health visitor, adistrict nurse team and a midwife attached to the practice. We also host PCNstaff.
Theclinical team is supported by the Practice Manager, a team of 8 Receptionistssupervised by a Reception Lead, 6 secretaries/admin supervised by a SeniorSecretary, a Medical Records Clerk, 2 Administrators and a Facilities Managerplus Apprentices.
We have a PMS contract to deliverservices to our patients and aim to achieve the higher standards and targets inthe Quality and Outcomes Framework as well as local targets.
We are in very pleasant purpose builtpractice owned premises. Our communication and remote working is facilitated byour computer network, and we use EMIS web for our clinical system.

Clinicalresponsibilities
Usingour GP E-consult online system to consult with patients and arrange face toface appointments with yourself according to need. This includes houseboundpatients and or their carers where the face to face appointment could be withyourself or may need to be arranged with an extended team member.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
In consultation with patients and in line with current practice disease management protocols, developing care plans for health
Providing counselling and health education
Referring to other care providers as appropriate
Prescribing in accordance with the local/practice prescribing formulary (or generically)/national guidelines whenever this is clinically appropriate
Issuing only computer-generated acute and repeat prescriptions via EPS whenever possible
To take part in an on-call duty rota with the rest of the GP team
Recording clear and contemporaneous consultation notes using agreed codes
To manage queries, results, paperwork and correspondence in a timely manner
Collecting data for audit purposes and contributing to the practice audit programme and clinical standard setting with the practice.
To carry out medical reports/examinations e.g. for insurance companies for patients you have been looking after and are best known to you.
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
