About the Transition to adulthood Service
Our Transition to adulthood Service supports young people aged 16-25 who are preparing to leave care or who have already left care.
We take immense pride in our role as corporate parents to the children and young people in our care, as well as those who have left our care. As Team Leader, you will lead a team of personal advisors, provide professional leadership, supervision and management oversight, ensuring practitioners deliver effective pathway planning, safeguarding support and transition arrangements that help young people achieve successful outcomes.
You will play a key role in maintaining high standards of practice, supporting workforce development and driving continuous improvement across the service.
