About the Transition to adulthood Service

Our Transition to adulthood Service supports young people aged 16-25 who are preparing to leave care or who have already left care.

We take immense pride in our role as corporate parents to the children and young people in our care, as well as those who have left our care. The Role

As Team Leader, you will lead a team of personal advisors, provide professional leadership, supervision and management oversight, ensuring practitioners deliver effective pathway planning, safeguarding support and transition arrangements that help young people achieve successful outcomes.

You will play a key role in maintaining high standards of practice, supporting workforce development and driving continuous improvement across the service.

What You'll Be Doing

• Leading, supervising and supporting a team of Personal Advisers.
• Providing oversight of pathway planning, assessments and interventions for care experienced young people.
• Ensuring statutory responsibilities under the Children (Leaving Care) Act and related legislation are fulfilled.
• Supporting practitioners with complex casework, safeguarding concerns and risk management.
• Promoting positive outcomes in education, employment, training, accommodation, health and emotional wellbeing.
• Championing young people's voices and ensuring they remain at the centre of all planning and decision-making.
• Building strong partnerships with Adult Social Care, Housing, Health, Education, Employment Services and community organisations.
• Monitoring quality, performance and outcomes to ensure services are effective and responsive.
• Supporting recruitment, induction, supervision and professional development of staff.
• Contributing to service development, inspection readiness and continuous improvement activity.
• Promoting a culture of reflective practice, innovation and learning across the team.

What We're Looking For

We're looking for a confident and inspiring leader, or someone ready to move into leadership,  who is passionate about improving outcomes for care experienced young people.

You will have:

• Significant experience within children's social care, leaving care or transition services.
• Strong knowledge of safeguarding, cared for and leaving care responsibilities and unaccompanied children seeking asylum.
• Experience of quality assurance, performance management and service improvement.
• Experience of supporting young people through significant life transitions.
• A thorough understanding of the challenges and barriers facing care experienced young people.
• Excellent communication, decision-making and organisational skills.
• Strong partnership-working experience across agencies and professional networks.
• A commitment to relationship-based practice and achieving the best possible outcomes for young people.
• Relevant professional qualification at degree level or equivalent.
