Pay Rate: £30.00 - £38.00 per hour

Hours: Full-Time (36 hours/week) | Flexible options available

What You'll Be Doing

As a core member of the team, you will be handling dynamic throughcare and comprehensive assessment work, ensuring vulnerable older adults receive the right support at the right time. You will:

• Conduct robust, person-centered assessments to determine care and support needs.
• Manage complex cases, ensuring seamless throughcare and transitional support.
• Safeguard vulnerable older adults while actively promoting their independence and well-being.
• Collaborate effectively with multi-disciplinary teams, health professionals, and families.

What We Need From You (The Non-Negotiables)

Because this role requires someone who can confidently manage a busy caseload independently from day one, we can only consider candidates who meet the following criteria:

• Qualified Social Worker: Valid registration with SSSC.
• Proven Experience: A solid background working within Older Adults, Adult Social Care, or Hospital Discharge teams.
• Assessment Expertise: Confident, proven skills in handling throughcare and complex assessment processes.
