We are a very busy Integrated Discharge Team, based in Whiston Hospital. We work closely with the Trust to ensure safe and timely discharges for patients and their families. We have a well established team in the hospital and work well with our colleagues on the wards. This is a fast pace working environment so if you think you have the skills come and join us!
We are an enthusiastic multi-disciplinary team, with the primary aim of ensuring a safe and timely discharge of patients across a number of local authorities.
We have recently joined the multi -disciplinary Contact Cares Team which provides additional opportunities for career development.
The work is diverse, challenging and rewarding, requiring individuals to be able to work independently and contribute to the working of a larger multidisciplinary team. The team is a mixed skill group with a range of experience who provide a supportive working environment.
This post is worked on a rolling rota basis, including evening, weekends and bank holidays. Additional allowances will be paid for shift and weekend working.
