Join our Dacorum Older People’s Team for a Team Manager to support people in a diverse locality.
About the role
You will lead and manage a Social Care Team. You will ensure that the team maintain a person-centred and strengths-based approach to improve outcomes for people through the completion of timely and effective Care Act assessments. You will ensure that the team provides an effective and robust service. You will work with the team to develop strong working relationships with local care providers and voluntary agencies to ensure that right services are being provided to the right people, at the right time. You will have oversight of the locality budget and will provide feedback to senior management on performance and development.
You will be part of the extra care panel to review at new tenants and ensure their needs are being met and they are appropriate for the service.
You will be working alongside partner agencies to support those within the area and attending the Complex Multi-disciplinary Team meetings and multi-agency risk assessment conference to discuss individuals referred.
About you
Essential:
- A qualified and registered social worker or occupational therapist
- Full driving licence and access to a personal vehicle for travel across the locality
- Proven experience in managing adult social care teams
- Strong crisis management, assessment and decision-making skills
- A passion for supporting people to remain safely in their own homes
- Excellent communication and partnership-building abilities
