What you'll do

• Coordinate person‑centred care delivery, setting priorities for the shift
• Safeguard dignity, privacy and independence at all times
• Support personal care and mobility using safe techniques
• Oversee hydration and nutrition; escalate concerns promptly
• Administer, check and reconcile medications accurately
• Provide companionship and lead meaningful activities
• Ensure cleanliness and safety checks are completed
• Recognise changes, complete observations and escalate to clinicians
• Communicate effectively with families and the MDT
• Maintain accurate documentation, handovers and task allocations
• Mentor, support and appraise junior carers
