Hospital Discharge Social Worker - Falkirk Council

We are seeking a dedicated and professional Hospital Discharge Social Worker to join a dynamic health and social care partnership. In this pivotal role, you will be responsible for bridging the gap between acute NHS healthcare and adult social care, ensuring a seamless, outcome-focused transition for patients moving from a hospital environment back into the community.

Responsibilities

• Conduct comprehensive, strengths-based assessments to identify the social care needs of service users and their families.
• Evaluate risks related to the home environment and daily living to ensure safe and timely discharge.
• Coordinate care packages, equipment, and home adaptations in collaboration with multidisciplinary teams, including nursing, physiotherapy, and occupational therapy staff.
• Apply statutory frameworks, including the Adult Support and Protection (Scotland) Act 2007 and Adults with Incapacity (Scotland) Act 2000.
• Maintain accurate case management records and conduct post-discharge reviews to ensure ongoing alignment with eligibility needs.
• Support service users in navigating Self-Directed Support (SDS) options to promote independence.

Qualifications

• Professional qualification in Social Work.
• Registration with the relevant professional body (e.g., SSSC).
• Proven experience in a social work setting, preferably within a hospital discharge or adult care environment.
• Strong understanding of relevant Scottish social work legislation and safeguarding protocols.
• Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary setting.

Conditions

• Pay Rate:£37-£38 per hour umbrella
