Adult Practitioner, Adults with Learning Disabilities and Autism Team, Telford and Wrekin
Rate of pay: £21.25
You will support the delivery of statutory Adult Social Care duties in line with the Care Act, Human Rights Act, and Mental Capacity Act. This role involves working closely with individuals, families, and carers to assess needs, carry out reviews, and develop person-centred support plans that promote independence, wellbeing, and informed choice.
We are looking for a dedicated and compassionate professional committed to delivering high-quality, person-centred support to adults and their families. You will have strong experience in Adult Social Care, particularly in completing reviews and reassessments, alongside the ability to carry out assessments and develop effective, outcome-focused support plans. A strong focus on review work is essential, ensuring care and support arrangements remain appropriate, responsive, and aligned to individuals’ evolving needs while promoting independence and wellbeing.
