Emphasis on completing Care Act Reviews and maximising health care funding Hours: Monday to Friday 9 am to 5 pm (37 hours) Pay rate: £31.97 Umbrella Length of assignment: To 31st August 2026 Working location: Hybrid – (office/client/home) Requirements: Enhanced DBS with Adults & Children's Barred List - Driver To manage a social work caseload, contributing to weekly peer reviews using an asset-based, community-led approach to inform case decisions by the Team Manager. To promote close and constructive relationships with individuals, their families and the local neighbourhoods. The post holder must be able to assess/support plan/review individuals receiving care and support from Adult Social Care with an emphasis on an Asset based approach. Liaise with colleagues in own and other council services and external agencies in order to gather information relevant to Assessment and Support Planning activities.
