Role Overview
Nottingham City Council is seeking a Personal Advisor – Leaving Care Team to provide high-quality, person-centred support to young people leaving care and help them successfully transition into independent adulthood.
The role involves managing a caseload of Children in Care and Care Leavers, developing and reviewing personalised Pathway Plans, and providing practical, emotional and developmental support to help young people build independence, resilience and long-term stability.
The successful candidate will work closely with young people, social workers and multi-agency partners to ensure care leavers receive the right support across areas including housing, education, employment, health and wellbeing.
Key Responsibilities
- Manage a caseload of Children in Care and Care Leavers, ensuring statutory responsibilities are met.
- Develop, implement and regularly review individual Pathway Plans based on each young person’s needs, goals and circumstances.
- Build trusting relationships with young people, providing consistent support, guidance and advocacy.
- Support care leavers to develop independent living skills, including budgeting, tenancy management and accessing benefits.
- Help young people access and sustain suitable accommodation, education, training and employment opportunities.
- Identify safeguarding concerns, complete appropriate risk assessments and escalate issues in line with safeguarding procedures.
- Work collaboratively with Social Workers, Housing teams, Education providers, Health services and voluntary organisations.
- Signpost and refer young people to specialist services including mental health, substance misuse and immigration support where required.
- Maintain accurate case records, assessments, reports and documentation in line with council procedures.
- Undertake regular visits and face-to-face meetings with young people to review progress and wellbeing.
Essential Experience
The ideal candidate will have experience of:
- Working with children, young people or vulnerable individuals within a social care, education or community setting.
- Supporting young people transitioning into adulthood and independent living.
- Managing a caseload and maintaining accurate case records.
- Developing support plans and reviewing outcomes.
- Safeguarding vulnerable young people and responding appropriately to risk.
- Working collaboratively with multi-agency partners.
Knowledge & Skills Required
- Understanding of Leaving Care legislation, including the Children (Leaving Care) Act 2000.
- Knowledge of Children Act 1989/2004 and safeguarding responsibilities.
- Understanding of Pathway Planning and support planning processes.
- Knowledge of key areas impacting care leavers, including housing, education, employment, benefits and wellbeing.
- Strong communication, relationship-building and advocacy skills.
- Ability to support young people in developing confidence, resilience and independence.
Qualifications
A recognised Level 3 qualification or above in a relevant area such as:
- Health and Social Care
- Children and Young People
- Education
- Community Support
Alternatively, candidates with demonstrable experience supporting children, young people or vulnerable individuals within a relevant setting will be considered.
Additional Requirements
- Full UK driving licence and access to a vehicle insured for business use.
- Ability to complete an appropriate DBS check.
- Ability to work flexibly, including occasional travel and work outside normal office hours when required.
