Bluestones Medical Agency is currently recruiting skilled and compassionate Band 5 Community Nurses to support Adult Community Services within Midlands Partnership University NHS Foundation Trust. This is an excellent opportunity for nurses seeking flexible agency work while delivering high-quality care to adults within community settings.

Key Responsibilities

• Deliver high-quality nursing care to adult patients within the community.
• Support individuals with acute, chronic, and complex healthcare needs.
• Carry out clinical assessments, care planning, and implementation of treatment plans.
• Administer medication and treatments in accordance with NMC guidelines and local policies.
• Work collaboratively with multidisciplinary teams, GPs, social care services, and families.
• Promote patient independence, wellbeing, and rehabilitation within home and community environments.
• Maintain accurate patient records and documentation.
• Ensure safeguarding procedures and patient safety standards are always upheld.

Requirements

• Registered Nurse (RGN or equivalent) with valid NMC registration.
• Minimum 6 months recent NHS or community nursing experience.
• Community or district nursing experience is desirable.
• Strong communication, clinical assessment, and organisational skills.
• Ability to work independently and as part of a multidisciplinary team.
• Full UK driving licence and access to a vehicle preferred.
